Admissions & ReferralsHow to Join or Refer a Resident
We accept self-applications as well as direct referrals from case managers, social workers, parole and probation officers, hospital discharge planners, and community organizations.
Application & Referral Steps
Submit an Application or Referral: Prospective residents can complete the online intake form. Agency partners can submit a client referral packet via email or our provider portal.
Initial Screening: A short conversation with the applicant and referring case manager to discuss goals, eligibility, and program expectations.
Intake & Move-In: Complete standard intake paperwork, review the house agreement, tour the facility, and transition into the home.
Information for Referring Partners
Direct Collaboration: We maintain regular communication with referring agencies, providing progress updates, attendance confirmation, and housing verification letters as needed.
Fast Intake Turnaround: Our intake team reviews provider referrals within 24 to 48 business hours to prevent delays in placement.
